Patient Responsibilities

  1. The responsibility to provide, to the best of the patient’s knowledge, accurate and complete information regarding past medical history and issues related to the patient’s health, including unexpected changes, to the individuals responsible for the patient’s health care program.
  2. The responsibility to make known the patient’s clear comprehension of the contemplated course of action and what is expected from the patient.
  3. The responsibility to follow the recommended treatment plan for the patient’s health care, including instructions of nurses and health personnel as they enforce applicable UHS rules and regulations.
  4. The responsibility to keep all appointments and notify UHS or the appropriate person when unable to do so.
  5. The responsibility of assuming the consequences of any decision to refuse treatment or to follow instructions.
  6. The responsibility to fulfill the financial obligations as promptly as possible.
  7. The responsibility to be considerate of other patients’ rights and those of UHS personnel in the control of noise, smoking, and number of visitors, and to be respectful of the property of other persons and of UHS.
