Patient Responsibilities

  1. The responsibility to keep all appointments and notify the student health center when unable to do so.
  2. The responsibility to provide accurate and complete information regarding past medical history and current issues related to their health to the best of their ability.
  3. The responsibility to ask any questions regarding their health, evaluation and treatment plan to insure understanding.
  4. The responsibility to follow the recommended treatment plan, as instructed by the health center professionals
  5. The responsibility to assume all consequences of any decision to refuse treatment or follow the recommended plan.
  6. The responsibility to ask questions concerning charges prior to health care services and to fulfill any financial obligations to the University for services provided.
  7. The responsibility to be considerate and respectful of other patients, guests, and UHS staff.
