Health Services for Graduating Students
Students who are graduating in May are eligible to be seen at the Health Center through May 23, 2008. If you entering a ND graduate program for the summer or fall semester, you may continue to use University Health services throughout the summer. Local Options: MedPoint, Memorial Hospital or Saint Joseph Regional Medical Center.
Medical Records and Immunization History
If you need copies of your medical records, come to Health Servicesr to sign the required consent for release of information, and we can have your records ready for pickup within 1 business day.
Pre-Employment and Travel or Service PHYSICALS
Please bring any pre-printed Physical forms to the Health Center prior to scheduling so that a nurse can review the form with you. You will be scheduled appropriately for the specific requirements of the form.
For students who cannot be accomodated at Health Servicese, we refer to Dr.Steven Simons at:707 E. Cedar St. South Bend. (574) 237-7890 or to Medpoint.
Pre-Employment DRUG TESTING
Below is a list of local agencies that will assist in collection of samples for pre-employment drug testing requirements. Fees listed are subject to change. Payment must be made at time of service. Then you may seek reimbursement from your employer.

Insurance
Graduating students who have purchased the school sponsored health insurance through Student Resources are covered until August 15th. You are eligible to receive care at the University Health Center through May 23, 2008. After that, you must consult your list of local providers provided in the brochure, or call 1-800-767-0700 to access a preferred provider.
For any medical care while you are away from the campus area, call Student Resources at 1-800-767-0700 to request a list of providers for that area. You may also access this information online. For medical providers outside of South Bend, in Indiana, go to www.CHAnetwork.com. Listings are provided for physicians and hospitals. For providers outside of Indiana, go to www.beechstreet.com.
Remember to carry your insurance card with you - it is printed on the back page of your insurance brochure. If you do not have it, stop by Health Services and pick one up before you leave campus.
