The Notre Dame sponsored Student Insurance Plan is an excellent
and affordable plan available to both undergraduate and
graduate students. Health Insurance for undergraduates is
encouraged but not mandated; however, graduate students
must be covered by a health insurance plan.
All registered degree seeking graduate and international students are
automatically enrolled in student insurance plan unless
proof of comparable coverage is provided. The premium of
the student health insurance plan is assigned to your student
account.
If health insurance is desired, student dependents have
a choice of an optional plan described in this booklet. Only
Notre Dame students may use the medical services provided
at the Health Center; however, dependents may have prescriptions
filled at the University Health Center Pharmacy according
to University policy and the benefits of their plan.
Note that no insurance plan provides 100% coverage, and
the University does not assume responsibility for any medical
cost incurred by students

Students are required to return a completed History and
Physical Form before they report for registration. Students
who have been accepted will receive a packet of information
through the mail, including this History and Physical Form.

Required Vaccinations by the State of
Indiana for college students include:
- Tetanus: Within the last 10 years.
- Rubeola (Hard Measles): (Exempt if
born before 1957). Must have had had two measles vaccines,
one after the first birthday, and one at least 28 days
after the first one.
- Mumps: (Exempt if born before 1957).
Two vaccinations.
- Rubella (German Measles): (Exempt if
born before 1957). One vaccination.
OR
- MMR: Must have two doses, the first
after age one, and the second at least 28 days after the
first dose.
Recommended Vaccinations include:
- Hepatitis B: Series of 3 vaccinations over a 6-month
period.
- Meningitis
- Pertussis - Adult booster

Proof of immunization may be provided through
the following options
- Written documentation by the practitioner who administered
the vaccine.
- Written documentation by the practitioner who diagnosed
the active disease.
- Immunization records kept at a previous school.
- Blood tests (titers) drawn to determine the presence
of antibodies to these diseases, demonstrating having
had them, with a copy of the lab resultsforwarded to Health
Services.
- Obtaining immunizations from this Health Center, your
own private physician, or local Public Health Department.
If the person is deficient in providing this documentation
they are prevented from registering for classes until the
requirements are met.
