Immunization Requirements
Vaccinations required by the State of Indiana for college students include:
- Tetanus: Within the last 10 years
- Rubeola (Hard Measles): (Exempt if born before 1957). Must have had had two measles vaccines, one after the first birthday, and one at least 28 days after the first one
- Mumps: (Exempt if born before 1957) Two vaccinations
- Rubella (German Measles): (Exempt if born before 1957) One vaccination
or - MMR: Must have two doses, the first after age one, and the second at least 28 days after the first dose
Recommended Vaccinations include:
- Hepatitis B: Series of three vaccinations over a six-month period
- Meningitis
- Pertussis: Adult booster

Proof of immunization may be provided through the following options
- Written documentation by the practitioner who administered the vaccine
- Written documentation by the practitioner who diagnosed the active disease
- Immunization records kept at a previous school
- Blood tests (titers) drawn to determine the presence of antibodies to these diseases, demonstrating having had them, with a copy of the lab results forwarded to Health Services
- Obtaining immunizations from this health center, your own private physician, or local Public Health Department
Failure to meet the immunization requirements may result in a delay in registration for the next semester.
