Pre-Registration Requirements

Students are required to return a completed history and physical form before they report for registration. Students who have been accepted will receive a packet of information through the mail, including this form.

If you are planning to register for fall classes, you should return the form by July 1 to:

Immunization Costs at Health Services

Proof of immunization may be provided through the following options

  • Written documentation by the practitioner who administered the vaccine
  • Written documentation by the practitioner who diagnosed the active disease
  • Immunization records kept at a previous school
  • Blood tests (titers) drawn to determine the presence of antibodies to these diseases, demonstrating having had them, with a copy of the lab results forwarded to Health Services
  • Obtaining immunizations from this health center, your own private physician, or local Public Health Department

Failure to meet the immunization requirements may result in a delay in registration for the next semester.
