Employee On-the-Job Injuries
Information for Notre Dame Employees
Notre Dame employees who are injured during the course of their work are to receive care through University Health Services. Our main concern and area of responsibility is the health and safety of the employee. It is not the responsibility of Health Services to determine if an injury will be covered under the worker’s compensation program. That determination is made by the Risk Management Department.
The employee must have a supervisor complete the “Report of Injury” form and should bring a copy to to the first appointment.
The registered nurse on duty will screen patients and provide care or refer patients to the UHS physician or emergency department.
Those who require emergency care are to be referred to St. Joseph’s Regional Medical Center.
