New Student Requirements

Welcome Letter

Once you’ve been accepted to Notre Dame and have received your NDID, please submit information about your 1) health history and 2) health insurance (if required) via the patient portal at nd.studenthealthportal.com any time after May 3. (This applies to new students for Summer and Fall 2024).                              

  1. Print and complete a University of Notre Dame Immunization Form and log into your patient portal to upload it.
  2. Enter your vaccine history and complete the online forms (Immunization Form, New Student Requirement Form, Communication Preference Form, Emergency Contact Form, and Under Age 18 Consent to Treat Form if it applies) between Friday, May 3 and Friday, July 5.
    • Failure to meet the immunization requirements may result in a delay in registration for the Spring 2025 semester and all subsequent semesters at the University of Notre Dame.
  3. Submit all completed health forms online between Friday, May 3 and Friday, July 5. (Summer & Fall 2024 classes.)
  4. Mandatory Insurance for Graduate, Professional, and all International Notre Dame students ONLY.  (This does not apply to Domestic undergraduates and you do not have to apply for a waiver.)
    • You are required to have health insurance coverage that meets University criteria.
      • Therefore, you are automatically enrolled in, and charged for the University sponsored health insurance plan.
      • However, if you have health insurance that meets University criteria, you may waive out of the University sponsored health insurance plan.
      • For the 2023-24 academic year, the insurance plan will go into effect on May 15 for NEW SUMMER students.  If you need it sooner, email insurancequestions@nd.edu.
    • You must use the enrollment/waiver application process if you want to waive the plan. 
      • Summer Students complete the waiver between May 15 and June 15 AND you will get an email when the site is ready.
      • Spring Students complete this process between January 1 and February 15 AND you will get an email when the site is ready.
      • Fall Students complete this process between July 20 and September 15 AND you will get an email when the site is ready.
    • If you fail to complete this process by the deadline, or if your insurance plan doesn’t meet University criteria, you will remain enrolled in - and charged for - the University sponsored health insurance plan.
    • If you have dependents, they are eligible to use the ND Wellness Center.  You need to complete the eligibility Wellness Center Form form and submit to your department. 
  5. The University of Notre Dame no longer requires undergraduate, graduate, and professional students to receive the COVID‑19 vaccine as a condition for enrollment. Consistent with the Centers for Disease Control and Prevention, the University continues to strongly encourage all members of the campus community to receive the primary vaccination series and the most updated booster dose.
