Return to Notre Dame

UHS: A crucial step on the journey back to Notre Dame.

At the time of your withdrawal from or return to Notre Dame, you may have been informed that UHS would be involved in your return process regarding your readiness to resume academic work.

The documents below comprise the information that will be required by UHS in order to provide a recommendation to an appropriate person, office, or program in the decision-making role regarding your return.  These documents may assist you and your treatment providers while you are engaging in health treatment during your time away from the University.

Undergraduate Students

Your initial return application must be submitted to the Readmission Committee in the Office of Undergraduate Admissions (link to admissions.nd.edu/) by the appropriate deadline listed on that website. After your initial application has been received and UHS notified, you will be contacted by UHS to print and complete the required UHS return forms and procedures outlined within them.  Links to the required UHS return forms are listed below.

If you are returning from a previous separation or withdraw, your materials must be returned no later than April 15 (if applying for the following fall semester) or November 1 (if applying for the spring semester).  If you are returning from a separation, UHS will be able to make a recommendation to the Readmission Committee ONLY after receiving all required materials.  The Readmission Committee makes the final decision regarding any undergraduate student’s return.

Graduate Students

You must notify both your program and the Graduate School of your intention to return prior to the semester in which you plan to return.  The Associate Dean of Students (ADS) of the Graduate School will notify UHS of your interest in returning.  You then will be contacted by UHS to print and complete the required UHS return forms and procedures outlined within them.  Links to the required UHS return forms are listed below.

If all coursework has been completed, re-enrollment occurs on a rolling basis throughout the year.  If coursework remains, the deadline for receiving the completed UHS return materials is no later than April 15 (if applying for the following fall semester) or November 1 (if applying for the spring semester).  UHS will be able to make a recommendation to the ADS ONLY after receiving all required materials.  In consultation with the ADS, the program makes the final decision regarding the graduate student’s return.

Law School Students

You must contact the Law School Admissions Office at lawadmit@nd.edu no later than April 15 (if you are eligible for fall semester return) or by October 15 (if you are eligible for spring semester readmission).  Please note that re-enrollment is not automatic.

The Law School will notify UHS of your interest in return.  You then will be contacted by UHS to print and complete the required UHS return forms and procedures outlined within them.  Links to the required UHS return forms are listed below.

Your readiness to return and resume the pressures of academic life at Notre Dame Law School will be evaluated by UHS and a recommendation made to the Law School.  The completed UHS return materials must be received no later than April 24 (if applying for the following fall semester) or November 20 (if applying for the spring semester).  UHS will be able to make a recommendation ONLY after receipt of all required materials.  The Law School makes the final decision regarding the student’s return.
